Shoulder Examination

Checklist
Wash hands, introduce yourself, consent patient.

  • LOOK
    • Scars, swellings, deformities
    • Muscle wasting
    • Scapular winging at rest
  • FEEL
    • Temperature
    • Bony prominences and joint: clavicle, GHJ line, scapular spine, AC joint
  • MOVE
    • Active, then passive movement and monitor for pain of:
      • Abduction (0-180)
      • Extension/Flexion (-30/180)
      • ER (flexed elbow adjacent to body) (0-90)
      • IR (reach hand behind back to contralateral scapula) (buttocks/lower back/scapula)
    • Power in all of the above
  • SPECIAL
    • Nerves:
      • In addition to median, ulnar and radial
      • Musculocutaenous
        • Sensory – lateral forearm
        • Motor – elbow flexion
      • Axillary
        • Sensory – lateral deltoid
        • Motor – abduction
    • Painful arc – rotator cuff
    • Empty beer can – rotator cuff
    • Speed’s test – biceps tendonitis
    • Yergason’s – biceps tendonitis
    • Apprehension – dislocation
    • Sulcus – dislocation
