Neurological Examination

Checklist
Wash hands, introduce yourself, consent patient.
Briefly assess for facial asymmetry, postural asymmetry, dysphonia and walking aids when gaining consent.

Lower limb
  • OBSERVATION
    • Gait – on toes, heels, feet in line
    • Squat
    • Romberg’s test
  • INSPECTION – scars, wasting, symmetry, fasciculations
  • TONE – ankle, knee, hip
  • POWER – toes, ankle, knee, hip
  • REFLEXES – Patella (L2-L4), Achilles (S1), Babinski (UMN lesion)
  • SENSATION – L1 Inguinal region, L2 Medial thigh, L3 Medial knee, L4 Medial Malleolus, L5 Medial Toe, S1 Heel
  • CO-ORDINATION – knee to foot along shin (dysmetria), foot to examiner finger (ataxia)
  • PROPRIOCEPTION
UPPER LIMB
  • OBSERVATION
    • Pronator drift
  • INSPECTION – scars, wasting, symmetry, fasciculations
  • TONE – wrist, elbow, shoulder
  • POWER – fingers/thumb, wrist, elbow, shoulder
  • REFLEXES – Biceps (C5/6), Brachioradialis (C5/C6), Triceps (C7/8)
  • SENSATION – C5 lateral shoulder, C6 anterior thumb, C7 middle finger, C8 little finger, T1 medial elbow
  • CO-ORDINATION – finger to nose, hand flip (dysdiadochokinesia)
  • PROPRIOCEPTION
